Regardless of the reason you are requesting a replacement, your departmental HR representative must initiate a Badge Request to authorize the printing of a replacement badge.  Please contact your HR rep prior to coming to our office.  (Request expires 30 days after issue date.)

Replacement Fees

  • Damage due to normal wear & tear
    (see Damaged Badge Replacements below)
  • Legal Name Change
  • Title Change
  • Department Change
  • New Position
  • New credential(s) - must be approved by department 
    as necessary for position
  • Lost or stolen
  • Damaged intentionally or due to misuse or neglect
    (see Improper Use/Care below)
  • Preferred First Name change
  • New photo when current badge is functioning properly

 

 

Card replacement fees will be charged to the U-Bill for students and will be payroll deducted for faculty/staff.

Damaged Card Replacements

Cards that are no longer working and have no visible defects or damage are considered "worn out" and will be replaced at no charge if the damaged card is surrendered at the time of replacement.  

Multiple "worn out" card replacements within a 12 month period may be subject to the $25 replacement fee as determined by University ID Card Programs staff.

Improper Use/Care

There will be a $25 replacement fee for cards that are damaged due to misuse or improper care.  Examples include, but are not limited to:

  • Vertical/horizontal gouges or scratches on the magnetic stripe
  • Bent or curling cards
  • Vertical break of card or jagged vertical/horizontal break
  • Abnormal marks including teeth marks
  • Equipment damage (such as from a washer or dryer)
DO NOT:
  • Store your card on the back of your phone
    • Keep it away from wireless phone chargers and contactless payment readers
  • Punch a hole in your card
  • Put a magnet next to your card
  • Bend your card
  • Use your card as a tool or device for which it is not intended
    • Example - Don't use it as an ice scraper

Cards exhibiting damage from any of the reasons listed above, will result in a $25 replacement fee which will be payroll deducted for employees and students will have the charge added to their U-Bill.
