For the convenience of UI Community Clinics staff and providers who work more than 50 miles from the UI Health Care Medical Center University Campus, the university is providing a convenient web application that will allow you to submit a photo online in lieu of being photographed in person.  Photos must meet the university's ID photo standards.

Before submitting a photo, please review the web page we have created with examples and requirements for submitting the perfect ID photo.  Photos must meet the university's standards and requirements and will be accepted or rejected based on that criteria.

Complete these four easy steps to submit your photo online:
  1. Review the university's ID photo requirements
    • Your photo should resemble a passport style photo (smiling is allowed)
  2. Login to our mobile friendly online card request application using your HawkID and password (you may be required to authenticate your login with Duo)
    • Make sure you select UI Health Care Badge as the request reason
  3. Upload your photo
    • Option 1 - Use a mobile phone or tablet to take a selfie (or enlist a friend to take it for you), or submit a photo already stored on your device
    • Option 2 - Use a computer to upload an original, digital image file (do not submit a photo of a photo) with a resolution of 150 dpi or higher
    • If you have had an ID with the university previously do not choose the current photo we have on file, we need a new photo of you
    • You're almost done when you reach the page with the card preview.  Select the "Submit Request" button to finalize your submission
  4. Watch your email for confirmation that your photo has been approved or rejected
    • In the event your photo is rejected, return to step #1 and submit a different photo
    • Once your photo has been accepted, you will no longer be able to upload any additional photos

Please make sure you submit your photo well in advance of the deadline specified to allow for sufficient time to resubmit a photo in the event your prior photo submission is rejected.  Once your photo is approved, it may take several business days for your badge to reach your clinic location and be delivered to you by your Supervisor, Manager or HR Representative.

Please note this method of uploading photos must be pre-arranged with University ID Card Programs and is an alternate method of badging for those employees working in clinic locations outside of a 50 mile radius from the UI Health Care Medical Center University Campus.
